The physicians of Orthopaedic Specialists have enjoyed providing exceptional orthopaedic care in Gaston County for many years.  Due to unforseen circumstances, our office will be closing on June 6, 2008.

If you wish to obtain a copy of your medical records prior to June 6th, please notify the office.  You will need to complete a release form. Your records will be copied at no charge.

If you wish to obtain a copy of your medical records after June 6th please send or fax a letter requesting your records to the company listed below. You will be billed by CopyMed for this service.

Please mail or fax your letter to CopyMed with the following information listed:

Date of request, patient's name, home address, date of birth and where you would like the records sent.
